 I agree with Fairy Lu, if you are touching yourself around these areas its best to wash your hand before you go touching anything else. If anything it is just good hygiene practice.

It depends, man. I mean, do you wash your hands after you urinate? Do you make sure that there are no more urine in your penis after you urinate? Do you wipe your penis after urinating?
It depends on your hygene really. If you switch underwear once or twice a week, then yes it is nasty. If you take a shower every day, change underwear every day, make sure you let all the urine drip out of you everytime you urinate, you wipe your penis tip with a small tissue everytime you urinate, and you wash your hands, then masturbating is not nasty. It's probably the cleanest place if you follow anything above.
I am not a professional. But you gotta keep an element of hygene, bro. It's for your own sake.
