I apologized and she said don't worry about it and nothing changed between us, but since then (3 days ago), we havent had sex yet and I think I screwed up our sex life, what should I do?

This should be no problem - let reasonable time pass and be more careful next time. Apologize if necessary, but continue to maintain it was accidental if true. Some of both genders are a bit sensitive about our rectal areas in youth, but many also become more receptive about some activities down there as time passes.

Go and kiss her and tell her you love her. Keep your hands on her waist and back and not near her bum.
Then make a move on her later but keep your hands on her top half as soon as the foreplay finishes.
Make her feel safe with you and that you are not after anything anal.
I personally feel that talking about this is going to be very awkward so actions may be better than words.
Good Luck!! xx
