The problem, whenever the massage therapist start working on my shoulders area, I get an erection which is embarrassing. So far, the massage therapist has not said anything. I pretend nothing is going on and hope the massage therapist don't realize that I have an erection. In the future, any advice on how to handle this situation?

I would suggest you insist on keeping on your underwear and wearing a towel over your private area. Even if the massage therapist is male, a professional remains professional; because it comes with the job. You're relaxed and that's how the body sometimes responds to touch and persistent stroking or caresses. You're very young and your hormones are super-active. It's a reflex or involuntary-response.
In time the mind tends to associate the touch as therapeutic; and automatically separates sensual from medicinal. Like when your genitals are fondled under medical-examination; your mind will recognize the purpose and discriminate between sensual and medical procedure. An erection is spontaneous and common among male-clients; and meaningless to the massage therapist anyway.

Think of your grandma. Then again, I dont think an erection is that unheard of or that the massage therapist cares.
