See a dentist, avoid smelly food and drink, don't smoke, brush you teeth and tongue, floss, and use mouth wash.
Chew gum after meals and before kissing!

Don't forget to brush your tongue too! That really helps get rid of the gross stuff that causes the bad smell.

Good hygiene is good hygiene. There's really no miracle plan to making your breath smell fresh as a daisy besides brushing your teeth (and your tongue, an often forgotten spot), eating mints or gum and using mouthwash. When you're at the store, get a travel sized mouthwash and carry it with you. It comes in handy!! Oh yes, and avoid onions, garlic, pickles, coffee and cigarettes if nothing else. They are delicious, but they can get you funky!!
