I have no idea where our lives are going and even whether there going in the same direction. How do I talk to him about this with out making him feel like I'm pushing him to get the operation?

Also instead of an operation or continuing to trying to conceive, why not adopt? If you are ready to provide a loving home for a child maybe take in a child that already exists and needs a good home.
