At break time at work when we are sitting around the table he will shake uncontrollably sometimes and I do not know how to respond. I would consider it rude to ask what is causing this. It tends to be his lower body. Does anyone have any ideas as to what could be causing him to tremble.. He is a heavy smoker and constantly goes for a cigarette break but I do not think this is the reason

Involuntary muscle spasms. Sometimes the muscles can go into a spasm.
There are many causes..
You just accept it as he cannot control them.
