Well the biggest(no pun intended) thing he has to worry about is Testicular Cancer. And if he's in doubt..he should check it out. :^)No but in all seriousness, there are a variety of reasons that he may have one testicle...1. He could've had or currently has Testicular Cancer. If he had it surgically removed, this may be the reason for his missing testicle. If he's not sure if he has it, he should really get checked by a doctor- and routinely perform self exams to check for lumps, abnormalities, etc. 2. Temperature. Temperature can affect a person's body. If cold is the issue, he should do something to stay warm. A raised/tight scrotum can give the illusion of having one testicle- and it could be because of the temperature. So by adjusting the temperature, this will enable him to have better blood circulation throughout his body, and allowing the testicles to hang more. 3. There's also a condition called Cryptorchidism. This means that one or both testicles do not descend into the scrotum. This is a condition that is usually caught at child birth. But if it's not spotted or corrected, one of the testicles will just wither away. The good news is many men can still function in and out of the bedroom with one testicle. They also have the capabilities during intimacy to produce sperm/fertilize eggs and maintain erections.

My brother-in-law was born with one testicle and my sister had three children and my brother-in-law had to have vasectomy when they decided three was enough.
