He forgot so much, so I ended it, but now I want him back.

Help me please, i just did something stupid. Well my boyfriend and I have been together for five years now. Recently, he started giving excuses with being together. He stopped texting,forgot my birthday and I got paranoid. I asked him why and he said work so I told him to get lost. That i never want to speak or talk to him again. I love him so much and am so missing him its driving me crazy. He wont answer my calls, read my mail or sms let alone reply. Am scared he wont see me when I go to see him. I have apologised and all but he won't burge. The other day, when I called him at work, he let phone ring then second time a lady answered. He knows am jealous and he would have done it to push me but I want him back and am so sad. Please help me. I dont know what to do and I dont want to loose him.
