If your partner is infected with HIV and the exposed part of your penis has broken skin, you could contract the infection.
If your partner has genital warts or herpes or a syphilis chancre, the exposed part of your penis could be vulnerable to these.
Fungal or bacterial infections can also be acquired on the exposed skin.
Chlamydia and gonorrhoea are less likely because they tend to infect via the urinary tract, but the tip of your penis was covered. However, you should still be tested for these, even if you don't develop any symptoms.
Chlamydia and gonorrhoea are tested by a urine sample. Herpes, syphilis, and HIV are tested by blood. If herpes-looking sores show up, these can be swabbed and tested. If a wart develops, it can be biopsied for testing. Possible fungal and bacterial infections are swabbed and cultured.
Remember that it takes weeks or months for HIV antibodies to develop. Your doctor will advise if you need additional testing in the future.

While the risk is reduced, yes it is still very possible to catch an STD with the condom applied this way.
Best thing to do next time is to occasionally check the condom to ensure that it is rolled all the way down to the base of your penis.
