 You seem to know exactly what you want, you're unhappy and want out of the relationship.
Make sure you do want this and its not something you can work on together, if it is still what you want then you have to make the break.
Talk to him, say what you think and feel and what you want to happen, I don't see if you're so sure you want out of the relationship why you can't seem to unless theres a reason you haven't shared.
You have to do what is right for you, no matter what if you're unhappy then you have a right to make the change that will make you happy.
