I have always been slender, but this is a noticeable difference from last time since my family and friends have been commenting and saying somewhat hurtful things about my lack of a 'figure'This is has led to worry that he'll be surprised and disappointed with me now that I look different.Do I tell him about it before he sees me, and mention my illness so that he's aware? It wouldn't hurt mentioning to him that you've been sick and its caused some changes to your body. Not for the sake of pleasing his curiosity, but more just in casual conversation.
I couldn't see it as being a problem and as long as you get on the right track to getting your body healthy again, there shouldn't be any concern. Good luck.

If it is a noticeable difference, he might well be surprised. I can't really think of anyone I know that wouldn't be surprised if they suddenly saw that their girlfriends had lost weight. It's not so much about the 'appearance' thing - it just might be that he's shocked to see you in a different way.
It's probably worth saying something, but even if he is shocked, don't think that it's personal. You'll probably find that he's even a little worried about you.
