In my experience the lady communicates her desires about this, and her partner learns to read her cues.
It varies from time to time, but my lady most often likes to have the head just inside her when she climaxes and she supplies all the movement. (Yes, getting disconnected can be a problem.) She likes a couple of long, slow strokes as she finishes - which occasionally triggers a mild second orgasm.

 Slow thrusting for me. The contractions make it intense enough that he is just inside me, to have him move in addition gets a bit too much, and also takes away some of the pleasure. At least I think so. I guess this is up to each and everyone to decide. But remind you, I haven't orgasm during sex from sex alone, I do it with a vibrator combined with the sex.
