Combine your hand around his penis and your mouth over the head and use them together in one motion, you can take a break when your jaw gets tired but keep your hand moving. Make up for your mouth not being there by extending the stroking of your hand to the tip. And keep the shaft well lubricated too. Mix it up a little until he lets you know that you've found the right combination and rhythm.
