 While masturbating to porn/celebrities is fairly common and is not usually a cause for undue alarm, doing so with other people one knows blurs the line a little. I would not call it normal unless there is an understanding between the people in the relationship to the effect that such behavior will be kosher.

 It's up to the individuals in a relationship. Not to sound like a therapist, but I don't like using the word normal in this case. It's common, but it (porn/pictures) can cause some problems in some relationships (not all). I don't think you'll find a relationship where neither person ever doesn't fantasize about someone else. Almost everyone fantasizes about other people from time to time. It becomes a problem when you NEED to fantasize about others to get off or find yourself regularly doing it while with your partner.

 it depends on how open minded the relationship is. There are no 'right and wrong' rules in general, the rules are placed between two people and how they communicate and how their specific relationship is.
Personally, I don't think I'd be okay with my boyfriend (if I had one) masturbating to girls they know. HOWEVER, I'd be totally okay if they masturbated to girls they didn't know, like to porn.
And to answer your next question, I'm sure it happens but I'd personally feel guilty about imaging someone else I knew personally while getting it on. That would tell me that I really should not be dating one person exclusively if I was interested in more than one guy to begin with.
