He went through some hard times and was depressed and I still haven't forgiven him for everything that went on and held the grudges. He apologized. I know he's sick of me. How do I apologize to him and change the way I act?

Only you can change the way you act. You just have to figure out why you do it and then stop it.
Take a breath and THINK before you say something nasty to him.
The fact you did it when he was down is pretty bad. Tell him how you know what you did was wrong and want the chance to change.
But if he leaves you then you may have to try and work this out alone so you don't mess up your next relationship too.
Good Luck!! xx
