If you masturbate to the porn try to do it elsewhere without the porn so it doesn't become something you need.

I applaud you sir.
It is really something that you even recognize the fact that you have a problem.
That with the fact that pornography is everywhere we look these days doesn't help matters.
It will not be an easy thing for you to get away from but I am going to give you some suggestions that I know work.
The first thing you need to do is if at all possible get your computer to some common area where someone can see what is on the screen any time they walk by. That means that if you have your computer in your room you may need to leave your door open and your screen visable from the door.
Another thing is make it a habit to turn off the monitor or even better yet the entire computer the minute you finish what you were originally doing on the computer in the first place.
In addition I am adding a website that I trust completely.
It is a religious site but don't draw conclusions before you see what the article has to say. It can really help and it sounds like you are serious about breaking away.

First, I think it's good that you're mature and self-aware enough to realise that looking at porn can be harmful and that there are better ways to waste your time. So, well done.
Porn addiction is often borne out of boredom. Try to avoid situations where you KNOW you'll look at porn. If your computer is kept in your room, move it somewhere public. If you know you look at it at a certain time of day, get out and do something. Go for a run, catch up with some friends. If you break the porn addict cycle now, you won't have to worry about it when you're older and you'll be thankful for it.
Count the number of days without porn and make a challenge out of it. You control your mind and although it will be a test of will, you can do it. The longer you go without porn the more your desire to look at it will drop (after a time).
Also, go out and meet girls. Get friends with them. Get involved in activities with them.
